The entity is required to provide the name, address, date of birth, social security number, and valid government issued identification for each of the beneficial owners.
A Beneficial Owner meets at least one of the following criteria:
(1) Who directly or indirectly exercises "control" over the reporting company, or
(2) Who directly or indirectly owns or controls 25 percent or more of the "ownership interests" of the Business
